• Request amendments to PHI you believe is inaccurate.
• Receive an accounting of certain disclosures.
• Request restrictions on how we use or disclose PHI.
• Request communications in a specific way or at a specific location.
• Receive a paper copy of this notice.

Complaints

You may file a complaint with our Privacy Officer or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
